living environment. Our houses are furnished, contain a combination of single and double bedrooms, have fully applianced and equipped kitchens, telephone service, peer supports and employment direction, shared bathrooms, television with cable or satellite, and all houses are located near bus lines.\n\nThe weekly program fee is $210.00\/week, and an admission fee of $210.00 is charged. Please see attached house rules which are incorporated into this contract, and include mandatory house and 12-step meetings. If all rules are followed, and a two week notice of departure is provided, the admission fee is refunded, otherwise the admission fee is not refunded.\n$6.00 PER WEEK IS REQUIRED FOR HOUSE DUES, PAID AT THE HOUSE, FOR TOILET PAPER, CLEANING SUPPLIES,\nPAPER TOWELS, DRUG TESTS, ETC. When a participant moves out for any reason, all personal property must be removed. By signing below as applicant, I authorize Sober Solutions to discard or give away any personal property I do not remove within 72 hours of moving out. In no event is Sober Solutions responsible for participant's personal property, and program participants are encouraged to secure their own insurance. I agree that if l am away from the house for more than twenty-four (24) hours without notice, I will be deemed discharged.\n\nIF YOU ARE ELIGIBLE FOR THE BEHAVIORAL HEALTH RECOVERY PROGRAM {BHRP}, IT WILL COVER 100% OF YOUR PROGRAM FEE, FOR UP TO SIXTY (60) DAYS. EACH PROGRAM PARTICIPANT MUST COMPLETE:<\/strong><\/p>\n\n        <ul>\n\n          <li><strong>TWO (2) CHORES PER WEEK<\/strong>.<\/li>\n\n          <li><strong>MUST KEEP THEIR PERSONAL AREAS CLEAN\/ORDERLY<\/strong>.<\/li>\n\n          <li><strong>PROGRAM PARTICIPANTS MUST PERIODICALLY HELP WITH MAJOR CHORES<\/strong><strong>, SUCH AS SPRING &amp; FALL CLEANUP, MAJOR HOUSE CLEANING, PAINTING, MOVING FURNITURE<\/strong>.<\/li>\n          <LI><strong>SNOW REMOVAL FROM WALKS AND STAIRCASES, ALONG WITH SPREADING ICE MELT, IS TO BE DONE BY OUR CLIENTS DURING AND AFTER EACH STORM. THESE DUTIES ARE CONSIDERED A CHORE.<\/STRONG><\/LI>\n\n        <\/ul>\n\n      <p><strong>10. A $210.00 APPLICATION\/ADMISSION FEE IS REQUIRED. IF ALL RULES ARE COMPLIED WITH, INCLUDING THE SIX MONTH COMMITMENT, THE ADMISSION\/APPLICATION FEE WILL BE REFUNDED<\/strong><\/p>\n\n        <p><strong>11. THE WEEKLY FEE IS $210.00 AND MUST BE PAID BY 4:00 PM FRIDAY, IN ADVANCE, FOR THE WEEK SATURDAY THROUGH FRIDAY. USE OF AN AIR CONDITIONER IS CHARGED FOR SEPARATELY AND IS NOT INCLUDED IN THE WEEKLY FEE. IF YOU ARE ELIGIBLE FOR THE SRHS (SUPPORTIVE RECOVERY HOUSING SERVICES) PROGRAM, IT WILL COVER 100% OF YOUR HOUSING COSTS, FOR UP TO SIXTY (60) DAYS, AND THE ADMISSION FEE WILL NOT BE CHARGED TO YOU UNTIL YOUR ELIGIBILITY TERMINATES.<\/strong><\/p>\n\n        <p><strong>12. ALL PROGRAM PARTICIPANTS ARE REQUIRED TO BE OUT LOOKING FOR EMPLOYMENT MONDAY THROUGH FRIDAY, FROM THE HOURS OF 9 AM TO 3 PM. CLIENTS THAT ARE NOT ABLE TO WORK, NEED TO FIND APPROPRIATE DAY TIME ACTIVITIES DURING THOSE HOURS, IE: VOLUNTEER WORK, GO TO THE LIBRARY, 12 STEP MEETINGS, ETC. SLEEPING OR WATCHING TV ALL DAY, IS NOT AN OPTION IN OUR HOUSES. CLIENTS WHO SECURE WORK, CAN WORK THEIR OWN SCHEDULES.<\/strong><\/p>\n\n        <p><strong>13. A LIMIT OF ONE CAR ON PROPERTY, WHICH MUST BE REGISTERED AND INSURED<\/strong><\/p>\n\n        <p><strong>14. TWO (2) WEEK WRITTEN NOTICE OF DEPARTURE REQUIRED FOR RETURN OF APPLICATION\/ADMISSION FEE. NOTICE PROVISION NOT WAIVED FOR PROGRAM PARTICIPANTS ASKED TO LEAVE FOR VIOLATING RULES<\/strong><\/p>\n\n        <p><strong>15. NOT REPORTING THAT SOMEONE IS USING IS GROUNDS FOR BEING DISCHARGED<\/strong><\/p>\n\n        <p><strong>16. SMOKING IS NOT ALLOWED IN ANY OF OUR HOUSES OR ON ANY OF OUR PORCHES. IN THE EVENT I DO NOT REMOVE MY BELONGINGS WITHIN THREE (3) DAYS, TIME BEING OF THE ESSENCE, I HEREBY AUTHORIZE THE DISCARDING OR GIVING AWAY OF MY BELONGINGS WITHOUT ANY COMPENSATION DUE TO ME.
